How we approach this kind of build

Health data raises the stakes on access control. Every record access is scoped by role and by care relationship, and every read as well as every write is logged — in a clinical system, who looked at a record is as auditable a question as who changed it. Non-production environments run on anonymised data so real patient information never leaves the production boundary.

The scheduling engine is where most of the operational value sits. Appointment types have different durations, clinicians have individual availability and leave, rooms and equipment are finite, and double-booking has to be impossible under concurrent requests. Automated reminders by SMS and email are the single highest-return feature, because missed appointments are pure lost revenue for a clinic.

Telemedicine runs over WebRTC with a signalling server we control, so consultations are peer-to-peer where the network allows and relayed only when it must be. The clinical notes interface is deliberately usable one-handed during a consultation, because a records system that is slow to fill in during an appointment gets filled in afterwards, badly, or not at all.
